(1) To determine their level of general knowledge/skills/disposition on X need for the job,
(2) To determine their level of technical knowledge/skills/disposition on X for the job,
(3) To determine their need for professional development on general knowledge/skills/disposition of X, and
(4) To determine their need for professional development on technical knowledge/skills/ disposition of X,
One of the needed section is to identify the background of your respondents.
*Important tips when making a survey - Survey must have title, Explain a bit about the survey (eg. the uses, state how many section is there in the survey, how long will it take), Encourage people to fill the survey( be polite), Show people how to respond and DO NOT make the survey complicated)
